As promised I am starting a new blog page. This is to continue our prior efforts but to make scrolling easier. We shall discuss PMCH ki andruni khabaren.
Good new for us. Three families from PMCH '89 batch are now settling in Houston. Reminiscent of " Kyunki saas bhi kabhi..."

Sid, how many computers do we have in our library and how well connected is PMCH library to the outside world. Do we have an institutional access to some of the databases essential for practice- like Uptodate, Ovid or Medline? If so, please confirm.

Everyone out here! Hope you all had a good time during this weekend. While I am gone to Patna, I am hoping to see RKP and a few other faculty members besides some of the folks who I am going to see through Sid who could give us constructive ideas about helping the institution as alumni. Do you have any thoughts on what we could accomplish as a group besides counselling people about further career options available.

Who should I be contacting if I want help/grants to be provided for getting some services to our institution.

I had a long chat with one of our PMCH alumni, Dr. Sonal Singh who could help in getting an institutional account of Uptodate for PMCH if SOMEONE from PMCH faculty could collaborate on this project. If we get this done, PMCH students could access latest unabridged information online. If we could pool in and help our institute get some other databases, I am hopeful that students will achieve maximum benefit from this.

Please share your thoughts.

Encouraging to see the website, but it looks amateurish in design and concept. Prognosis does not look too great. The information displayed is latest from 2006 making this website 2 years too late! But you can direct your energies into other avenues for now.

as for computers in library! there r 7 of them, but they r not very well networked resulting that 2 of them dont belong to the LAN.
nobody knows abt MEDLINE......i think 5 years ago dr shakeel ahmad( the then health minister) put up one computer with VSNL internet connection and MEDLINE subscription inside the storage room of library, NOBODY used it.
later on 1 year ago 7 computers were setup in area overlooking ganges, north of librarian's office.
heard that dr vps sir is visiting USA.!!!!

i was thinking of putting up posters and notices regarding a USMLE Orientation programme (with you)
i talked to the principal, he gave me the permission for use of audi.
but i think we decide the dates first for it. we'll have to prepapre a slideshow abt USMLE and RESIDENCY; and then a open Q/A session.

We could do it that way depending on the number of people we will have for the meeting. Otherwise we could just talk about everything one on one.

Can we use one of the LTs instead of the audi as you would just need to get the keys from Ramashish ji (is that name right, the LT staff?), if that could be done without much effort? Either of the places, we would need a Laptop/computer with projector.

About Uptodate we could start off with a trial version, which allows you 60 days of unlimited access from any of our institute computers. It is a something similar to Harrison, but far more user friendly and if you need to look for something in real quick time, you could go online and look up the information. Now I understand that we have 7 computers in the library, but we don't have internet access on all of them? How are those computers best used by the students around?

dont worry abt crowd! i think we can easily attract more than 100 students if they r informed beforehand. the downside is that 2004 and 2005 batch r having univ exams, but those who r really interested can squueze their schedule for 2 hours....
the thing is publicity........i'll try my best for it....posters in library, canteen, hostels, i am spreading the word by mouth, in orkut, etc.
in audi we have lcd projector, but in lt we dont have one working.....we'll have to ask from some department to release it for few hours. (difficult!). * rampreet ji*
actually the internet connection burned out( expired) and new connxn is being applied from months....lets hope we get it real quick!
if the guys there renetwork the lib computers then all would work easily. but for that we need a specialist hardwarre networking guy!
uptodate is like what? i dont get it. is it that thru it i can access full text article of journals. PUBMED gives only abstracts withg few free fulltext. if uptodate can enable us to view all jourtnal articles full text , then it would be boon for all! mainly for PGs who have to work for their thesis, and guys interested in research!
better to buy uptodate when internet connection is established!

Hi Israrul

I and Hemant were the first students in PMCH's history to take step 1 during our 3rd year. Even though you may think that it was an achievement. I would discourage you in taking step 1 before completion of your final MBBS exam. Most of the questions in step 1 are clinical based.

My recommendation is to do well in your 1st & 2nd MB's and keep reading the high yield stuff as outlined in those websites.

Your knowledge is important but you have to be street smart for these tests as well.

It helps to have a clinical background, plus taking the exam so early is not going to help as you still need step 2, CSA etc. So plan your dates carefully. Scores are very important and should be a priority. I have heard Kaplan is a good material to start from. As far as individual books are concerned, a book by Vikas Bhushan i.e. First Aid to step 1 and 2 grades books by their strength and popularity. You could all books recommended as A in this category.

Hi Firdaus,

Confusion is alright since different people have different takes on the right time to appear at this test. Conventionally we have been taking the test in our intern years or after as we have more clinical exposure and more knowledge base to be able to successfully take step 1. If you really plan on taking the test after 2nd MB which would be approximately 2 and a half years from now, you will have to prepare for the test smartly. You should be solving test questions from the Pre-test series for each subject that comprises the USMLE step 1. You should at the least have Kaplan Q bank (I guess you get that pirated from Delhi) to solve subjectwise qs and you must keep doing it often. The closer you get to your targetted date to take the test, you will have to prepare more intensely using resources like USMLE World q bank and Kaplan resources. Having a study partner or a group of people who have similar goals would be helpful. You will have to be sincere to your textbooks (not necessarily the teachers)during your semesters.

If you use your resources well, keep yourself focussed and determined to achieve the best, you can always do it. Best of luck!

Who said it was difficult to take step 1 in y'r 2-3rd MB.

You defenitely can do it....provided you score at the percentile current candidates are scoring. Remember nowadays passing the USMLE is not the goal.....but scoring in the 90's is as I understand(Abhay correct me if I am wrong).

I took step 1 in 1994, at that time getting a score of 80 was considered very good, but in todays era that will not get you a single interview. I am not discouraging you all but just trying to help you to make a right decision.

Once you have a score...that's it! you cannot correct it...remember!!

If you are confident and doing well in your Q banks....go for it!!

I am sorry, I have not been able to write a lot of what I have wanted to for various reasons. I still hope to write all of it sometime soon though. Just to reassure you, it is a worthwhile cause. Whether to earn more money (it will be a while before 1 Re is worth 40 ddollars) or get more into academics or to get both of them if you are lucky. As far as the match process goes, it is a long route and tests your patience and asks for a lot of finances as well, but believe me every time that you will travel on an interview trail, it would be an enriching experience.

I also come from a family which did not have any allopathic doctors (my father is a homoeopath, but that is another story)practitioner, but with some information and lots of support, I was able to cover the whole distance. Same ingredients would help you- best scores, best possible medical education from your college (and you would have to try for it), information, skill to use this information and of course support from family in words and money both.

If you do it diligently, you will enjoy the process and the result both. Wait for more to come.

I am back after a long hiatus. I visited India after 3 yrs and got very nostalgic in PMCH. I appreciate Sid's effort in organising my meet with PMCH undergrads. I enjoyed the discussion with approx 30 some guys. Also good to see that there is more awareness, though I felt majority had very little information about the process and the setup.

in my opinion Vikas Bhushan provides a good list of books which are easy to read and high yield. I would not recommned reading basic text books we use for our MBBS exam.

aapiusa is making strong efforts to establish externship programs for FMGs especially from India. I have sent my affirmative mail to the coordinating committee. I would suggest all the PMCH guys in US to support this cause as well.

Many such websites have surfaced on web and seem to entice the FMG desperately trying to seek residency. I would caution all of you, as these companies charge quite a lot of money but the yield is very poor.

This though seems to be a very novel idea, except physicians have used this concept to suit their needs that is make money rather than provide guidance, NO OFFENSE...

i was looking for 4th yr clerkship programs in us universities.i have applied in couple of those..but a few univ wanted the students to have "COMPLETED CORE CLERKSHIP IN MED,SURG,OB-GYN AND PED." Now, can u tell me wat they are refering to??? is it just the ward duty we do here or something else..

i am applying to "u of louisville,kentucky...yale at Connecticut..harward(plz dnt laugh)" although i hav a list of univ...can you recomend a few (u knw the scenario there!!!)

Dear Manish,
getting a clerkship in a US university is an excellent idea. I think you should apply to all university hospitals. Since the application process is costly, please keep two things in mind:

1 - Many of the reputed programs have their core clerkship set up with many foreign medical school as part of international representation. I know, AIIMs, MANIPAL in India, AGA KHAN in Pakistan and Tel Aviv Univ in Israel have this wonderful option for their graduates. If possible I would explore uncharted territories, for obvious reasons.
2 - Apply in a location where you can hang around for at least 3-6 months. I would recommend applying in a locality where you know family, friends or even distant contacts. If not try to apply to programs where you would like to get residency.
3 - I will recommend you do serious preparation before you start your clerkship. Any of us on pmch.us main web page will be more than willing to answer your questions.
4 - Regrading your question pertainig to core clerkship; it refers to the number of hours you have spent in ward service after completing first MBBS. The universities require you complete certain number of hours in each faculty as Medicine, Surgery etc prior to starting internship. or more details, refer to the web site I have listed below.

For Saurabh:

the California letter refers to PTAL i.e Postgraduate Training Authorization Letter. California has its own rules and regulation regarding application. It's setup is parallel to ECFMG requirements, and now they have added finger print . There are god and bad things about getting PTAL:
1 - The process is very tedious and a pain in the neck. It is costly, very elaborate and multiple requirements have to be met. Because of this I did not undertake this process.
2 - However I recommend doing this simply because, not every one will take this painful route and the applicant pool becomes small. However the main attraction is the fact that California has a very large number of hospitals and your application has more opportunities. Also the state is kind to FMGs. Lastly at your stage when you are in campus, getting the paperwork done is easy.
3. There are many web sites that explain PTAL. I found this one useful: http://usmlein.com/usmle/?page_id=74


Final thoughts and some support for us people. When ever we need documents signed by office of Principal we have to deal with the local staff, who demand moderate financial returns. Can you guys set up a process which is easier and not as costly. When you join residency and fellowship and start your practice, all of you will need many document from the office of the principal. So if you can set up a process now it will be very useful, down the road.

Also, I have many USMLE books in pdf format, which I am going to send with Sanjeev. We would like to establish a system, where any willing applicant can access these books and if needed make photo copies. Any suggestions are welcome.

I am applying only to those univ that do not ask for non-refundable application fee ( avg $100/ elective).they may or may not ask for tuition fee($500 to 3200$ per month).this cut-shorts my list.

Another thing that cut my list short.....rather shredded my list as well as hopes was Dr.shiela Sharma (hod obgyn).i need a letter signed by her about completion of core clerkships. She listened only to half of what i had to say...and later drew a conclusion that wat i am up to (clerkship) is illegal. govt of bihar does not approve this and neither would she...blah blah blah. Wasn’t able to argue with her (u all know how things go around here...hod se panga...never!!) dont know wat to do??Waiting for sanjeev boss ...maybe , if he introduces Dr.V.Prakash to this alien thing called elective...i stand a chance.

what ppl say is that kaplan editions dont mattermuch. if u got latest then go for it, if not then dont waste time hunting for it coz in india many shops are selliong old editions by just changing the cover and editioon page as new editions.....i know ppl getting 99 with 2004 editions last year.
first aid is indeispensible. i advise you to make notes in first aid as u read thru kaplan and supplementary books. that way u'll have just onwe book to revise days b4 exam. FA has ample space in its pages to make notes.

yesterday sanjeev boss addressed us in the lt1...............he came right on the time and shown his slides that he has prepared especially for us.......shared his xperience and his journey to the us.........i mean to say that how much these seniors r doing for us to see us there............now its our part of the duty to live up to their xpectations.......i abstracted one thing from it is that sharing information is the key to success..........this forum is a good one and i request everyone not only those who have made it and also those who r going for it to share the information whatever u know..............maybe sometimes u can also b benefited

i have also checked for ECFMG medical school web portal(emswp).It costs subscription fee of 500 USD($) per annum for medical school.It costs nothing for institution like our college,but i m n't sure college will show interest in bcoz college finds difficulty in paying 39 rupees per day for daily basis
worker of wards and hostel of college.
What can we do for this?

Thx to sanjeev boss for spending some of his precious time with us.His meeting was very much interactive with all the students present at the lecture theature.The number of students was not great which i think is due to lack of intrest towards USMLE in our college.we must hope this will definately improve as number of students getting sucess in USMLE increases.For achieving this milestone i think pmc alumni US is doing great job.I would like to congratulate and thank sanjeev boss for his sincere effort.

Ater the meeting i thought it will be more helpful if someone who has recently took the steps could just tell us the details of materials and methods of study.

I am glad we are able to generate so much interest and enthusiasm.
One would take step 1 when you are ready for it. We used to have a guideline i.e when you are taking mock questions and scoring around 70% you will score between 80-85 percentile. This pertained to USMLE Q Bank questions. So in my opinion when you doing well in these mock question, you should take the test. Also it is not a good idea to study more than 3-4 months for these exams.
Dear Rajeev, I reviewed the web address, and frankly even institutes like AIIMS do not have a god presence on the web. I think the purpose is to identify the Alma mater, than to provide details of an institution. Frankly PMCH should have a dedicated web site, but there is a long list of "should" things to be done.

Finally we are not recent graduates, but one can easily find the resources you are looking for. Listen, there is no magic book that will get you the best score. Read recommended books, read small books that you can revise and solve as many q banks as possible.

There is a opening in Baylor to work in an autonomic lab ( to test for autonomic nervous system). One should be able to run the lab independently and should have working visa. The perks are:
you get to meet the faculty at Baylor , neurology and participate in the daily rounds and case conferences. Please spread the word around.

Sorry for the delay. I came back to US day before and in the process of unwinding all I gathered there after 8 yrs.

I had a wonderful time in PMCH and thanks to everyone who arranged the meet and were present.Those who could not make it...this portal is yours...use it! PMCH has changed for the better. I gave two lectures to the PG students & staff one in medicine & one in ENT. Dr.Vijay Prakash has made amazing changes to the department and I believe that in time to come he is going to impprove it. He is planning to incorporate DNB courses and fellowships as well.

As some of you suggested I spoke to Dr.Sheila Sharma on the eve that I had to leave patna, infact I went to her nursing home and met her....explained to her the need of students doing electives in US and that she could provide NOC's for that. She understands and has assured me of doing the same. Those of you who intend to do so can and get signed by her.

After talking to you all these were my observation/recommendation & thoughts:-

1. There is still a lot of miscommunication/no-communication between students who intend to take/have taken USMLE and others who have very little information.
Rec: Students should create a support group (call any name you want) - meet every week/fortnight - share your thoughts - Online groups are best but all may not have net access. Our site pmch.us is available for this - use it!


2. Share your ideas - People who know more about USMLE and current trends should share their thoughts through any means.....As I said to you all......people who know less-shed your inhibitions and find people and go up to them and ask.....'there is no question that is stupid'; people who have already done a lot of research and know well about USMLE- share!

3. Talk to the principal office regarding the ERAS official I talked about.

4. In your freetime start working on research/posters/papers(look at the format I gave you)....once again there is no journal that is unworthy.....a publication is a publication....wherever it is.....Its better to have some rather than nothing and waiting for the journal which is pubmed listed.

5. Regarding the current trend of electives being done in US-its a very brilliant idea to pursue that here and get good LOR's. Most of us who are here are at community hospitals and are private practicioners while the electives are available at university hospitals. If you want attachment in my hospital(http://www.anmedhealth.org/) I will be happy to help you in that. The only flip side to the electives is its a costly endeavour. Make sure you check with your family before you go ahead. Apart from the insurance you will need anywhere from $1200-$2000 per month to sustain here.

You guys are really doing good job. I am in Burlington, MA....(91 batch)

Here is my advice for USMLE;

1. Study Kaplan metarial ( at least four-five times) + first aid (4-5 time) + MCQ (Q bank + USMLE world (2-3 time))------then take exam ..........You will score 90+

2. Do not do various book consult.

3. Research is difficult to do in Patna. Try to publish case report or case series or some article in college Journal .......find some case report in journal read it and get idea. Then find good and interesting case ask your attending if they have good case ----- write it up send it to Journal. Similarly review article can be publish. These type of publication ...you are the one doing every thing and not dependent on other resources.

4. Several Medico from south India they first come USA to do MPH/ Master course. While they are completing MPH they take USMLE and finds residency. MPH is always plus point.
